High blood pressure not controlled by telmisartan or amlodipine alone, or as a convenient single tablet for someone already taking both.

Amlodipine relaxes the artery walls by keeping calcium out of their muscle cells, so the vessels widen. Telmisartan blocks angiotensin II, so the vessels are not being tightened in the first place and the kidneys release salt and water. The two paths add up, and telmisartan also relaxes the veins, which drains the fluid that amlodipine tends to leave in the ankles.

Dizziness, ankle swelling, flushing, headache and tiredness. Ankle swelling is usually less than with amlodipine alone.

Not for use in pregnancy. Avoid grapefruit juice, which raises amlodipine levels. Kidney function and potassium are checked with blood tests, and potassium supplements or salt substitutes should be avoided unless your doctor agrees. Stand up slowly at first, and never stop the tablet suddenly on your own. It is a prescription medicine in Pakistan.
